RN, Hospice/Palliative Care — On-Call — Unique opportunity for experienced nurses with long term care and/or end of life care experience. Earn full-time salary and benefits and work every other week! Requires current CT RN license and one-year long term care or end-of-life care experience.
Home Care/Hospice Case Managers — Full and part-time, with weekend opportunities. Provides direct care and coordinates interdisciplinary care to group of patients and families. Job share and flex hours possible.
RN, Home Care QI/PI — Flexible, part-time opportunity for experienced RN for clinical EMR document review/processing and staff education. High level of proficiency in standards of practice, regulatory and accreditation requirements in home health care required.
LPN, FT and Per Diem — Full time and per diem opportunities available for qualified LPNs in home health and hospice. Conduct patient/family visits under guidance of RN Case Manager in lower Fairfield County, including Greenwich, Stamford, Darien, Norwalk, New Canaan, Wilton and Westport. Shared weekend and holiday rotation; excellent compensation and benefits. Requires current CT LPN license and minimum of three years broad clinical experience, including one year of home care. EMR experience required.
Physical and Occupational Therapists — Per diem opportunities to join high-performing rehab team, with particular need for Stamford/Greenwich territory. Requires current CT license and three years experience; home care experience preferred.
Home Health Aides / Homemaker / Companions — Hourly, live-in and shift assignments available. Paid mileage and travel time. Home Health Aide positions require six months recent experience working as CNA or HHA or recent graduate of a Connecticut training program with certificate. Offering part-time and full-time positions available with benefits.
Intake Representative — Act as the initial clinical point of entry for referrals, questions, and requests for information from all sources. Develop and coordinate all pre-admission plans of care. Process all inquiries/referrals for agency services, including securing clinical, financial, and demographic information. Act as liaison between agency and the community. RN or LPN license (Preferred)
