Moderna COVID-19 Vaccine Clinic

Waveny/Visiting Nurse & Hospice will be holding Moderna COVID-19 vaccine clinics at the Visiting Nurse and Hospice offices located at 22 Danbury Road in Wilton on Wednesday, Sept. 22nd and Thursday, Sept. 23rd. Masks are required to be worn when receiving your vaccine. Vaccines will be distributed at The Visiting Nurse and Hospice Offices located at 22 Danbury Road, Wilton.

Click here to download the Consent/Registration Form – You must bring this completed form with you, along with a copy of your insurance card and photo ID, to the vaccine clinic in order to receive a shot. If you do not have insurance, you will still be able to receive the vaccine.

The vaccine clinics are scheduled as follows: Wednesday, Sept. 22nd from 11:00 A.M. – 2:00 P.M.; Thursday, Sept. 23rd from 1:00 P.M. – 4:00 P.M. and will be administered by registered nurses from Waveny/Visiting Nurse & Hospice. Each vaccine recipient will be monitored for 15 minutes after the injection and released by the registered nurses on staff.

Please be advised that you must be 18 years or older to receive the Moderna COVID-19 vaccine and you should not have received any other vaccine in the last 2 weeks. We will be offering 1st dose Moderna vaccines as well as 2nd dose Moderna vaccines – but only if your 1st dose was received at least one (1) month prior and you bring your vaccine card with you.

We will also be offering the 3rd dose of Moderna for those that are immunocompromised.  Please wear a mask, bring your vaccine card and a note or prescription from your physician verifying that you are eligible to receive the 3rd dose, these are required.
