Care Management is a service provided by Visiting Nurse & Hospice of Fairfield County that enables individuals to remain in their homes safely and comfortably. A professional Care Manager will develop and monitor a customized plan-of-care to meet the specific needs of the individual and his or her family. The goal is to optimize the individual’s ability to enjoy good health and quality of life while remaining at home in safety and in comfort.
What does a Care Manager do?
A Care Manager is a specialist who assists individuals and their families with the development, implementation and monitoring of a comprehensive care plan that allows for the greatest degree of independence and safety for individuals and enables them to enjoy the familiar comforts of home.
Who can benefit from Care Management?
Those who can benefit from Care Management are family members and caregivers as well as individuals who live alone and/or may need assistance with any of the following:
- Managing daily activities
- Nutrition counseling, meal planning and meal preparation
- Maintaining personal hygiene
- Seeking companionship
- Has lost a spouse or has no family living nearby
- Risk of falling
- Memory loss
